Family Partners are active members of the Case Management/Wraparound team processed by providing peer mentoring and support to primary caregivers
Introducing the family to the treatment process
Modeling self-advocacy skills
Providing information, referral, and non-clinical skills training
Assisting in the identification of natural/non-traditional and community support systems
Documenting the provision of family partner services, including both face-to-face and non-face-to-face activities
Collaborate with individuals in service to develop an individual care plan that is grounded in the family members’ perspectives
Participate in interdisciplinary team staff. Present psychosocial assessment findings.
Follow up with the individual in service within 24 hours on the assigned individual in services who access emergency services
Must possess a valid driver’s license and have reliable transportation to make community visits
Follow up on assigned individuals in service who missed scheduled appointments. Document attempts to contact individuals. Report repeated unsuccessful efforts to contact the treatment team
