Homeward Bound, a behavioral health agency for adults, is hiring a mental health peer specialist to work on the Deflection/Crisis Respite Unit where residents may stay for 1-14 days while they get stabilized due to being off of medication, experiencing housing instability, or needing coordination with other types of care but without the acuity of needing a locked down facility.
Candidates must have a valid driver’s license, have current liability insurance, and either hold current peer specialist certification or be willing to be trained as a peer specialist.
Job duties will include
Assist residents with all case management needs including, but not limited to, appropriate housing upon discharge, connection with positive social support, and coordination with necessary resources.
Coordinate care between deflection/crisis respite and mental health outpatient and other aftercare providers.
Complete intake documentation with clients including financial information.
Prepare residents in deflection/crisis respite for discharge and work with the treatment team to eliminate barriers to success.
Assist in direct care on the unit including observation, checking in and out belongings, facilitating phone calls, and attending meals.
Facilitate psychoeducational groups as needed and complete documentation.
Provide therapeutic intervention in potential crisis situations and assist the team in ensuring the appropriate level of care and coordination of services is provided.
Attend all treatment team staff meetings.
Be willing to take and document vitals for clients as needed.
Be trained in self-administration of medication and give medication to clients when needed.
Drive residents to appointments and stay with them during the appointment
Take residents to their discharge locations
Pick up residents’ medications as needed
Assist other departments in the facility with their transportation needs as needed
Job Type: Full-time
