Certified Peer Specialist / Case Management

Recovery Council is looking for a Certified Peer Specialists to join our Recovery Support Services (RSS) team.

This program will increase the prevalence and quality of long-term recovery for those individuals by enhancing quality of life, and increased social connections through sustained engagement in long-term recovery support services and to provide peer provided “Reach-in” support during incarceration to ensure incarcerated individuals with opioid and stimulant use disorder successfully transition from the county jail, prisons, and all other rehabilitative settings, into clinically appropriate community based care and other services.

· Align medication for opioid use disorder treatment services (MAT) with a recovery-oriented approach.

· Support individuals’ efforts to initiate and sustain their recovery.

· Expand the availability and quality of recovery-oriented supports and services, which may include digital support tools including tele-recovery.

· Rapidly identify newly incarcerated individuals with opioid and/or stimulant use disorders and connect them to a Certified Peer Specialist through in-person or telehealth services.

· Leverage the power of the peer experience to provide “Reach-in” support to engage justice-involved individuals in a mutually empowering relationship in order to support successful “Re-entry” transition from incarceration to the community.

· Reduce the risk of relapse and overdose to substance use and promote recovery through clinically appropriate services combined with long-term Certified Peer Specialist involvement.

· Increase stable community tenure, decrease recidivism, and promote recovery.

· Increase collaboration between the Local Mental Health Authority (LMHA), Medication Assisted Treatment (MAT) providers, and other clinically appropriate community-based services.

· Provide “Re-entry” peer and appropriate clinical supports to individuals within the first forty-eight hours after release, in keeping with national best practices for overdose prevention.

· Establish collaborative partnership with local sheriff(s) and jail(s), as well as, warden(s) and prison(s) and other rehabilitative settings.

· Identify incarcerated individuals who meet the requirements of the target population by establishing a referral system.

· Utilize the state’s CMBHS documentation system
