Recovery Support Peer Specialist

The Recovery Support Peer Specialist (RSPS) provides case management, peer counseling, crisis intervention, and education for participants in Corazón’s Recovery Support Services program. Drawing on lived experience, the RSPS fosters trust, models hope, and supports participants in building Recovery Capital through individualized recovery plans, case management, peer support groups, follow-ups, inductions into MOUD treatment, stigma reduction, and family support. This role coordinates care with community partners while ensuring timely data entry and case documentation in CMBHS in alignment with our grants contract.

Essential Duties and Responsibilities

Case Management and Care Coordination- Oversee individualized case management and care coordination for participants across outreach, residential, and outpatient settings. Provide crisis intervention, and motivational support to encourage engagement and retention in care. Utilize telehealth and digital tools to enhance participant engagement and follow-up.

Direct Peer Recovery Services- Provide one-on-one peer recovery coaching (face-to-face, telephone, virtual, or traveling companion). Support participants in developing and updating strength-based, person-centered Recovery Plans. Facilitate recovery support and psychoeducational groups addressing relapse prevention, coping strategies, family recovery, and wellness. Conduct regular recovery checkups and goal-setting sessions to monitor participant progress.

Indirect Recovery Support and Community Navigation- Coordinate access to  MOUD/SUD treatment, emergency shelter, housing, employment, transportation, childcare, food assistance, veteran services, and other indirect supports. Conduct documented follow-ups to verify linkage and outcomes of referrals. Develop relationships with community organizations to expand referral pathways and strengthen recovery networks.

Education, Advocacy, and Family Support- Deliver or coordinate educational services on addiction, recovery, employment readiness, life skills, and digital literacy. Provide stigma reduction and family support training to promote understanding of recovery as a lifelong process. Support participants and families in navigating systems such as probation, healthcare, and social services.

Data Entry, Reporting, and Compliance- Maintain detailed case notes, service records, and participant progress in CMBHS and Be Well Texas reporting platforms. Ensure timely submission of monthly performance data by the 5th of each month. Conduct and document Assessment of Recovery Capital (ARC) at enrollment, six-month, and twelve-month intervals. Uphold confidentiality and ethical standards in all documentation and communication.

Collaboration and Team Support- Work closely with the Recovery Support Services Program Manager and Senior Director to meet contractual performance outcomes. Participate in team meetings, supervision, and professional development trainings. Represent Corazón Ministries at community meetings, health fairs, and recovery events. Model trauma-informed and recovery-oriented principles in all interactions.

Qualifications:

TX Driver’s License and Reliable Transportation
Certified Recovery Support Peer Specialist (Active RSPS)
Proficiency in Microsoft Office Suite and data management systems.
Minimum of two years of sustained recovery from substance use.
Experience with care coordination, case management, or peer navigation.
Proficient in Recovery Capital assessment, and facilitator of Recovery Support Services (RSS)

Work Environment:

This position involves both office work (60%) and fieldwork (40%), including street outreach and community engagement activities.
Occasional evening or weekend work may be required.
Regular participation in community events and training sessions is required.
